In accordance with Texas Senate Bill 12 (SB12), a parent or guardian must provide written permission before a student may participate in any club. This form is required for all student clubs in Grand Prairie ISD, including curricular, co-curricular, extra-curricular and competitive clubs.
- If a club has prerequisites for membership (such as grade eligibility, applications, courses, etc.), those requirements must be met before the student can be accepted as a member.
- Completing this form does not enroll a student in the club. Students must still contact the sponsor or attend meetings to join.
- This form provides parental permission only.

National Honor Society
Spring 2026
Eligible seniors will receive an invitation letter on Friday, February 6 during their 1st, 5th, or 7th period class. Applications are due Friday, February 20, by 3 pm.
National Honor Society Is By Invitation Only: Eligibility for NHS is based on a student’s GPA. Juniors in the Spring semester or Seniors in the Fall semester will be contacted if they meet the minimum GPA requirement of 3.66 or higher.
Being a member in this organization is considered an honor. Membership includes a time commitment of scheduled meetings as well as volunteering at various events. To be considered for membership, you must complete the online application. You are also expected to obtain 5 teacher recommendations (using the same link). Make sure to attach the Community Service Log with your online application. Be sure to make a copy, edit the document, and upload completed document. There are hard copies located outside G244 and K402.
